RFT has its own state-wide multi-disciplinary assessment and planning team. This team works collaboratively with consumers and other key stakeholders to create consumer-centred plans. The plans include the consumer’s own goals and suggestions for how these can be achieved.
The team employs specialist skills in mental health, nursing, occupational therapy and social work, as well as additional specialist services where required. The plans are created using the Recovery Star – a holistic tool which helps address all key areas of a person’s life, including:
- Managing mental health
- Managing physical health
- Daily living skills
- Addictive behaviours
- Social networks
- Identity and self esteem
- Work, education and training
- Trust and hope
Plans are written in plain English and become the property of the consumers.
RFT’s planning and assessment services can be accessed by anyone.
Free to current RFT consumers. Fees may apply for service provided to people not currently in RFT programs.
